CMH Pediatric and Adolescent Services
Welcome to CMH Pediatric and Adolescent Services. The members of this staff are dedicated to providing comprehensive medical care for children and adolescents up to 18 years of age. In addition to providing the highest quality care for your child, we will work with you to help your child develop to his or her fullest potential. We offer a variety of printed materials that cover common pediatric concerns and issues relating to nutrition, growth and development, and common illnesses. The staff welcomes your questions and suggestions to improve service. We look forward to providing the best healthcare possible.

Appointment Information
Patients are seen by appointment only Monday through Thursday. Please arrive 10 minutes before your scheduled time. Same day appointments are available for sick children. When you call for an appointment, please tell the receptionist the reason for your visit. This is important for scheduling the proper amount of time for each visit. If you must cancel an appointment, please give at least 24 hours notice since someone else may need to see the doctor. Sometimes families bring additional family members to be seen during an appointment. It is difficult to care for more people than scheduled, so if more than one member of the family is being seen by the doctor, please advise the staff when making the appointment. Each new patient will be asked to complete a registration form for emergency numbers, billing information and insurance coverage. Please bring your insurance card.
