Appointments

All patients are seen by appointment Monday through Friday, 8:30 a.m. to 5 p.m. Please call (937) 382-5438 to make an appointment or for more information. When you call for an appointment, please tell the receptionist the reason for your visit and your referring physician. This is important for scheduling the proper amount of time for each visit. If you must cancel an appointment, please give at least 24 hours notice since someone else may need to see the doctor. Each new patient will be asked to complete an information form. Please bring the completed form with you. Each new patient will also be asked to complete a registration form for emergency numbers, billing information and insurance coverage. Please bring your insurance card.
