Care Management
Care Management is a free service offered by CMH Regional
Health System for you and your family. Case managers and social
workers make up a team that helps you and your family cope with
problems and stress that you may have while ill or in Clinton
Memorial Hospital. A case manager usually visits patients during
a hospital stay.
If you need a case manager, please call the Care Management office at 937.382.9318. The office is located on 4 East Tower, across from the waiting room.
Office Hours:
- Monday through Friday—7:30 a.m. to 4:30 p.m.
- Weekends—A case manager is in the hospital to meet new patients and take care of urgent needs.
- Holidays—A case manager is on call to take care of urgent needs.
