Advance Directives

International HealthAdvance Directives are legal documents that let you tell those that are providing healthcare what you would like to have done for you if you are ever in a situation where you are unable to express your wishes. These documents are recognized throughout Ohio and in most other states.

There are two main types of advanced directives: a Living Will and a Power of Attorney for Health Care. The Living Will applies in end-of-life treatment situations and lets you state your wishes regarding which medical treatments you would like to have performed or withheld for yourself.

The Power of Attorney for Health Care lets you choose who will speak for you and make medical decisions on your behalf if you are unable to make the medical decision yourself. This person can be the same or different from the person who you select to be a financial Power of Attorney but the paperwork is different.

The details about the circumstances, treatments and procedure for creating a Living Will or Power of Attorney are available through CMH Care Management. Ask your nurse to contact them for more information.
