Concerns or Compliments

During your visit with Clinton Memorial Hospital we will strive for excellence of care and customer service. If by chance you are dissatisfied with your experience, have comments, questions or ideas for improvement we encourage you to let us know as soon as possible. You may talk to your nurse, physician or other caregiver to provide assistance or if you prefer you are welcome to contact our CMH Patient Relations Representative at (937) 382-9207. Clinton Memorial Hospital has a Bioethics Committee, a Risk Manager, HIPAA Privacy and Security Officers, an Environment of Care Committee, an Infection Control Practitioner and an Employee Health Practitioner to support the goals and duties of the hospital and to resolve conflicts in care or customer service.

Patient Satisfaction Surveys

Following your visit to Clinton Memorial Hospital you may receive a survey in the mail about your visit. Clinton Memorial Hospital contracts with a company to conduct the surveys on our behalf. This company sends us a comprehensive report of the completed surveys with comments our patients have regarding their stay or visit. We use these surveys to better understand what our patients like and where our opportunities for improvement are. Please take a few moments to complete the survey if you receive one.

Additional Concern and Compliment Information

If you need help with a medical decision, first discuss the matter with your physician, clergy or other health care givers. If the decision is particularly challenging for you, the Clinton Memorial Hospital Bioethics Committee is a resource to patients, families, physicians and staff. The Bioethics Committee is not a decision-making body. It is an objective multi-disciplinary committee designed to assist evaluate, offer advice and act as an educational forum. You may request a consultation or review by submitting a request to the Chairperson of the Bioethics Committee.

You may ask your nurse for the name and number of the Bioethics Committee chairperson or contact the switchboard operator for information. The chairperson of the Bioethics Committee, or a member of the committee, will make every attempt to address an issue with you as soon as possible. Discussions with and recommendations made by the Bioethics Committee are confidential.
